What is Vesicovaginal Fistula Repair?

Vesicovaginal fistula (VVF) is an abnormal connection between the bladder and vagina. In developed countries, the most common cause is pelvic surgery (hysterectomy 80-90%); in developing countries, prolonged labor.

Continuous vaginal urinary leakage is the characteristic symptom. Diagnosis uses cystoscopy, vaginal examination, the methylene blue test, and CT/MR urography.

Surgical repair can be performed transvaginally (O'Conor technique), transabdominally, or laparoscopically/robotically. An interposition flap (omentum, Martius flap) increases the success rate.

Symptoms

Continuous vaginal urinary leakage
Recurrent vaginal infection
Perineal skin irritation
Urinary leakage starting in the early postoperative period
Psychosocial distress

Risk Factors

Repair failure (5-15%)
Bleeding and hematoma
Urinary infection
Ureteral injury (during repair)
Vaginal shortening

  • If leakage continues after fistula repair
