What is Radical Orchiectomy (Testicular Cancer)?

Radical inguinal orchiectomy is the removal of the testis together with the spermatic cord at the level of the internal inguinal ring through an inguinal (groin) incision in testicular cancer. It is both diagnostic (histopathological diagnosis) and therapeutic.

Testicular cancer is the most common solid tumor in men aged 15-35. It is divided into two main histological groups: seminoma and non-seminoma (embryonal carcinoma, yolk sac tumor, choriocarcinoma, teratoma).

The inguinal approach is mandatory; a scrotal approach increases the risk of tumor seeding. Pre-operative tumor markers (AFP, beta-hCG, LDH) and post-operative staging CT guide treatment planning.

Symptoms

Painless testicular swelling or hard mass
Sensation of heaviness in the testis
Asymmetry of the scrotum
Gynecomastia (related to elevated beta-hCG)
Back pain (retroperitoneal lymph node metastasis)

Risk Factors

Risk of injury to the contralateral testis (very low)
Hematoma
Wound infection
Inguinal nerve injury (ilioinguinal)
Psychosocial effects

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Immediate urology referral when a painless hard testicular mass is noticed
  • If swelling or growth in the testis develops
  • If post-operative scrotal swelling or fever develops
  • If tumor markers rise during follow-up
