What is Radical Prostatectomy?

Radical prostatectomy removes the entire prostate, seminal vesicles and, when indicated, the pelvic lymph nodes in localised prostate cancer. Robot-assisted laparoscopic prostatectomy (RALP) is currently the most widely used technique.

Nerve-sparing surgery, which preserves the neurovascular bundles, is critical for postoperative recovery of erectile function. Bilateral nerve-sparing is performed in suitable patients.

Positive surgical margins, Gleason score, pathological stage and PSA kinetics guide decisions about adjuvant or salvage therapy.

Symptoms

Localised prostate cancer (T1–T2, selected T3)
Intermediate- or high-risk disease with >10-year life expectancy
Rising PSA after active surveillance
Biopsy-proven clinically significant prostate cancer
Salvage surgery after failed radiotherapy (selected cases)

Risk Factors

Erectile dysfunction (nerve-sparing preserves 50–80%)
Urinary incontinence (mostly transient, 5–10% long-term)
Bleeding and transfusion requirement
Anastomotic stricture (1–5%)
Lymphocele after lymphadenectomy

  • At diagnosis of localised prostate cancer for multidisciplinary discussion
  • If PSA rises after surgery (biochemical recurrence)
  • If persistent incontinence develops after the operation
