What is Urethroplasty (Urethral Stricture Surgery)?

Urethroplasty is the surgical repair of urethral stricture. Excision and primary anastomosis (EPA), dorsal onlay graft urethroplasty, or flap urethroplasty techniques are used depending on the length and location of the stricture.

EPA is the gold standard in short strictures (<2 cm bulbar urethra) with a 90-95% success rate. In long strictures, buccal mucosa (cheek mucosa) graft is the most commonly used substitution material.

The most common causes of urethral stricture are trauma, iatrogenic (catheterization, endoscopic procedures), idiopathic, and lichen sclerosus (BXO). Urethroplasty provides superior long-term success compared with repeated dilation or endoscopic urethrotomy.

Symptoms

Weakened urinary stream
Difficulty voiding (need to strain)
Recurrent urinary tract infection
Urinary retention
Splashing (spraying of the urinary stream)

Risk Factors

Stricture recurrence (5-15%)
Erectile dysfunction (5-10% in bulbar urethroplasty)
Urethral fistula
Wound infection
Pain and numbness at the buccal mucosa donor site

  • If the urinary stream is progressively weakening
  • If acute urinary retention develops
  • If recurrent urinary tract infections occur
  • If the stricture recurs after dilation or urethrotomy
