What is TURP (Transurethral Resection of the Prostate)?

TURP (Transurethral Resection of the Prostate) endoscopically resects prostatic adenoma through the urethra in patients with benign prostatic hyperplasia (BPH) and lower urinary tract symptoms (LUTS) unresponsive to medical therapy.

Prostate tissue is cut in chips using monopolar or bipolar electrosurgery and flushed out through the bladder. Bipolar TURP uses isotonic irrigation and eliminates the risk of TUR syndrome (dilutional hyponatraemia).

It remains the gold standard for prostates of 30–80 mL. Larger glands are better managed by holmium laser enucleation (HoLEP) or open prostatectomy.

Symptoms

BPH symptoms refractory to medical therapy
Urinary retention (catheter dependence)
Recurrent urinary tract infection
Recurrent BPH-related haematuria
Bladder stones secondary to BPH

Risk Factors

Retrograde ejaculation (60–90%, most common long-term effect)
Bleeding (intra- and post-operative)
Urethral stricture (5–7%)
Urinary incontinence (1–3%, usually transient)
TUR syndrome (monopolar technique, <1% with bipolar)
