What is Varicocelectomy (Varicocele Surgery)?

A varicocele is an abnormal dilation of the pampiniform venous plexus of the testis. It is the most common correctable cause of male infertility and occurs on the left side in 85–90% of cases.

Microsurgical subinguinal varicocelectomy is the gold standard: a 3–4 cm inguinal incision with an operating microscope allows precise ligation of spermatic cord veins while preserving the artery and lymphatics.

Surgery is indicated for clinical varicocele with abnormal semen parameters and infertility. Semen parameters improve in 60–70% of men and natural pregnancy is achieved in 30–50%.

Symptoms

Infertility (abnormal semen analysis with a clinical varicocele)
Testicular pain or heaviness
Testicular atrophy
Palpable dilated scrotal veins ('bag of worms')
Testicular size asymmetry in adolescents

Risk Factors

Hydrocele (most common complication, 5–10% conventional, <1% microsurgical)
Recurrent varicocele (1–5%)
Testicular artery injury (very rare in microsurgery)
Wound infection
Scrotal haematoma

  • When a varicocele is detected during infertility work-up
  • If testicular pain or heaviness develops
  • If adolescent testicular size asymmetry exceeds 20%
  • If scrotal swelling or worsening pain follows surgery
