What is Urethral Sling Surgery (Stress Urinary Incontinence)?

Mid-urethral sling surgery is the placement of a polypropylene mesh tape under the mid-urethra to provide urethral support in stress urinary incontinence (SUI). TVT (Tension-free Vaginal Tape) and TOT (Transobturator Tape) are the most common techniques.

TVT is placed via a retropubic approach and TOT via a transobturator approach. In both techniques the mesh is positioned hammock-like under the urethra following the tension-free principle.

The long-term success rate is 80-90%. The procedure can be performed as a day case. It is the most common incontinence surgery in women.

Symptoms

Stress urinary incontinence (leakage with cough, sneeze, or exertion)
SUI unresponsive to pelvic floor strengthening exercises
Mixed urinary incontinence (with a predominant stress component)
Intrinsic sphincter deficiency
Incontinence after a prior hysterectomy

Risk Factors

Bladder perforation (3-5% with TVT)
Urinary retention (overly tight tape)
Mesh erosion (in the vagina or urethra)
Groin pain (obturator region with TOT)
De novo urge incontinence
