What is Urethral Diverticulum Treatment?

Urethral diverticulum is a cystic pouch structure formed by the urethra extending into the periurethral tissue. It is much more common in women. It usually develops as a result of infection and rupture of the periurethral glands.

MR imaging is the most sensitive method in diagnosis, clearly showing the size, location, number, and relationship of the diverticulum with the urethra. Voiding cystourethrography and urethroscopy support the diagnosis.

Treatment is surgical excision in symptomatic patients. Diverticulopexy (marsupialization) or complete excision can be performed. Although the risk of adenocarcinoma developing within the diverticulum is rare, it has been reported, and pathological examination of the surgical excision material is important.

Symptoms

Dysuria (painful urination)
Recurrent urinary tract infections
Dyspareunia (painful sexual intercourse)
Postmicturition dribbling (drop-by-drop urine after voiding)
Palpable mass in the anterior vaginal wall

Risk Factors

Urethrovaginal fistula formation (1-5%)
Recurrent diverticulum (5-10%)
Urinary incontinence (stress incontinence)
Urethral stricture
Wound infection

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • If recurrent urinary tract infections and dysuria are present
  • If swelling or mass is felt in the anterior vaginal wall
  • If drop-by-drop urine comes after voiding
  • If stone or malignancy is suspected within the diverticulum

Treatment Methods

01
Transvaginal diverticulectomy (complete excision)
02
Three-layer closure technique
03
Marsupialization (in selected simple cases)
04
Antibiotic therapy (in acute infection period)
05
Postoperative urethral catheter (2-3 weeks)
