What is Robotic Partial Nephrectomy?

Robotic partial nephrectomy (RPN) is the removal of renal tumors with a healthy tissue margin and repair of the remaining kidney parenchyma using the da Vinci robotic surgical system.

Trifecta (negative surgical margin + minimal kidney function loss + complication-free recovery) and pentafecta (trifecta + warm ischemia <25 min + no positive surgical margin) are success criteria.

It has become the standard treatment for stage T1 renal tumors (<7 cm). 3D imaging, augmented reality, and indocyanine green (ICG) fluorescence guidance enhance surgical precision.

Symptoms

Renal mass (cT1a: <4 cm, cT1b: 4-7 cm)
Bilateral renal tumors
Renal tumor in patient with single kidney
Hereditary kidney cancer syndromes (VHL)
Patients in whom kidney function preservation is required

Risk Factors

Bleeding and transfusion (2-5%)
Urinary fistula (1-3%)
Positive surgical margin (2-4%)
Warm ischemia injury (long clamp time)
Need for conversion (<5%)

  • If postoperative fever, bleeding, or urine leak develops
  • If deterioration in kidney function tests occurs
  • If recurrence is suspected on follow-up imaging
