What is Retrograde Intrarenal Surgery (RIRS)?

Retrograde intrarenal surgery (RIRS) is the advancement of a digital flexible ureteroscope through the urethra, bladder, and ureter into the renal collecting system to directly visualize stones and treat them with laser lithotripsy.

Holmium or thulium fiber lasers are used to dust stones (dusting technique) or to fragment them into small pieces removed with a basket. With 270 degrees of active deflection, all parts of the kidney, including the lower pole, can be reached.

EAU guidelines recommend RIRS as a first-line option for kidney stones 1-2 cm in size. It has advantages over ESWL and PCNL in obese patients, those on anticoagulants, and patients with anatomical anomalies.

Symptoms

Kidney stones (1-2 cm)
Lower-pole kidney stones (where ESWL success rates are low)
Stones resistant to ESWL (high density)
Kidney stones in children
Bilateral kidney stone treatment in the same session

Risk Factors

Infection and sepsis (1-2%)
Ureteral injury or stricture (1-3%)
Residual stone fragments
DJ stent-related symptoms
Damage to the flexible ureteroscope (cost)
