What is Renal Artery Stenosis Treatment?

Renal artery stenosis (RAS) is the narrowing of the kidney artery. Atherosclerosis (90%) and fibromuscular dysplasia (10%) are the two main causes. It can result in renovascular hypertension (1-5% of all hypertension) and ischemic nephropathy.

Atherosclerotic RAS is usually seen in elderly patients with cardiovascular risk factors, while fibromuscular dysplasia is more common in young women. Diagnosis uses Doppler ultrasonography as a screening test, CT/MR angiography, or conventional angiography.

Balloon angioplasty has high success rates in fibromuscular dysplasia. In atherosclerotic RAS, medical therapy (ACE inhibitor, statin, antiplatelet) is the first step; revascularization (stenting) is performed in selected cases. The CORAL and ASTRAL trials showed that stenting did not provide overall superiority over medical therapy.

Symptoms

Resistant hypertension (uncontrolled with 3 or more drugs)
Sudden-onset hypertension (in young or elderly patient)
Progressive deterioration of kidney function
Flash pulmonary edema (bilateral RAS)
Abdominal bruit (on examination)

Risk Factors

Renal artery dissection or occlusion (during intervention)
Cholesterol embolism (in atherosclerotic patients)
Contrast nephropathy
Stent restenosis (10-20%)
Retroperitoneal hematoma (arterial access site)

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • If hypertension cannot be controlled with multiple drugs
  • When unexplained hypertension develops at a young age
  • If renal function deteriorates after starting an ACE inhibitor
  • If episodes of flash pulmonary edema recur
