What is Intradetrusor Botulinum Toxin Injection?

Intradetrusor botulinum toxin (onabotulinumtoxinA) injection involves cystoscopic injection of toxin into 20-30 sites of the bladder muscle (detrusor) using a fine needle to reduce overactive bladder contractions.

Standard dose is 100 U for idiopathic overactive bladder (OAB) and 200 U onabotulinumtoxinA for neurogenic detrusor overactivity (multiple sclerosis, spinal cord injury).

Effect begins within 2-4 weeks and lasts an average of 6-9 months. Repeat injections can be performed when efficacy diminishes. It is a second-line treatment for patients who do not respond to oral antimuscarinic therapy.

Symptoms

Refractory overactive bladder (OAB)
Urge incontinence (unresponsive to antimuscarinics)
Neurogenic detrusor overactivity (MS, spinal cord)
Interstitial cystitis/painful bladder syndrome
Idiopathic detrusor overactivity

Risk Factors

Urinary retention (5-10% in OAB, 30-40% in neurogenic bladder)
Urinary tract infection
Transient hematuria
Need for clean intermittent catheterization (CIC)
Diminished efficacy over time (very rare)

When to See a Doctor?

  • If urinary incontinence persists despite medical therapy
  • If inability to urinate develops after injection (retention)
  • If fever or dysuria occurs after injection
  • When efficacy duration ends, for repeat injection planning
