What is TESE/Micro-TESE (Surgical Sperm Retrieval)?

TESE (Testicular Sperm Extraction) is the surgical removal of tissue samples from the testis to retrieve sperm in azoospermic men. Micro-TESE is an advanced technique that increases the sperm-retrieval rate by selectively removing dilated seminiferous tubules under an operating microscope.

In non-obstructive azoospermia (NOA), micro-TESE is the gold standard; sperm-retrieval rates are 30-50% with conventional TESE compared to 50-60% with micro-TESE. In obstructive azoospermia (OA), the sperm-retrieval rate exceeds 90% with both methods.

Retrieved sperm are injected into the egg via ICSI (intracytoplasmic sperm injection). Excess sperm can be cryopreserved for later use.

Symptoms

Non-obstructive azoospermia (NOA)
Obstructive azoospermia (OA)
Fertility desire after vasectomy (epididymal/testicular sperm)
Sperm retrieval in Klinefelter syndrome
Sperm retrieval in ejaculation disorders

Risk Factors

Testicular hematoma
Transient drop in testosterone
Wound infection
Testicular atrophy (very rare, lower risk with micro-TESE)
Possibility of finding no sperm (40-50% in NOA)

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • When semen analysis shows no sperm (azoospermia)
  • For urological evaluation during infertility workup
  • If post-procedure swelling or increased pain develops in the testis
  • For timing of sperm retrieval in IVF/ICSI planning
