What is Sacral Neuromodulation (InterStim)?

Sacral neuromodulation (SNM) is an implantable neurostimulation therapy that regulates bladder function by electrically stimulating the S3 sacral nerve root. InterStim (Medtronic) is the most commonly used device.

A two-stage approach is used: first, test stimulation (1-2 weeks) is performed to evaluate the patient's response; if there is more than 50% symptom improvement, a permanent generator is implanted.

It is indicated in patients with overactive bladder (OAB), urge incontinence, non-obstructive urinary retention, and chronic fecal incontinence who do not respond to medical therapy.

Symptoms

Overactive bladder (refractory to medical therapy)
Urge urinary incontinence
Non-obstructive idiopathic urinary retention
Chronic fecal incontinence
Interstitial cystitis/painful bladder syndrome

Risk Factors

Lead migration (5-10%)
Infection (2-5%)
Pain at the implant site
Device malfunction
MRI compatibility limitations (resolved with newer devices)

  • If urinary incontinence severely impairs daily quality of life
  • In non-obstructive retention with catheter dependency
