What is Pyeloplasty (UPJ Obstruction Repair)?

Pyeloplasty is the surgical repair of ureteropelvic junction (UPJ) obstruction. The Anderson-Hynes dismembered pyeloplasty is the most common and successful technique; the stenotic segment is excised and the renal pelvis is anastomosed end-to-end with the ureter.

UPJ obstruction may be congenital (most common in childhood) or acquired (after stone, infection or surgery). An aberrant lower-pole vessel compressing the ureter is a common associated finding.

Robotic pyeloplasty has become the standard approach in adults. Success rate is over 95%. In children, the open or laparoscopic approach is chosen according to age and surgeon experience.

Symptoms

Hydronephrosis due to UPJ obstruction
Recurrent flank pain (especially after fluid intake)
Recurrent urinary tract infections
Renal colic episodes
Loss of renal function (obstructive nephropathy)

Risk Factors

Anastomotic leak (2-5%)
Recurrence of stenosis (2-5%)
Urinary tract infection
Bleeding
Stent-related symptoms

When to See a Doctor?

  • Urology evaluation when hydronephrosis is detected on ultrasound
  • If there are recurrent flank pain and urinary tract infections
  • If fever or severe pain develops after pyeloplasty
  • For follow-up of renal function after surgery
