What is Peyronie's Disease Surgery?

Peyronie's disease is a connective-tissue disorder in which a fibrous plaque forms in the tunica albuginea, leading to penile curvature, pain, erectile dysfunction and difficulty with intercourse. It affects 5–10% of men.

The disease has an active (inflammatory, 6–18 months) and a stable (mature plaque) phase. Surgery is performed only in the stable phase, when curvature has been unchanged for at least 3–6 months.

Surgical options are shortening procedures (plication of the convex side), lengthening procedures (plaque incision/excision plus grafting) and penile prosthesis placement (in associated severe ED).

Symptoms

Palpable hard plaque in the penis
Marked curvature on erection (>30°)
Pain on erection (in the active phase)
Difficulty with intercourse (penetration problems)
Erectile dysfunction

Risk Factors

Penile shortening (1–2 cm with plication procedures)
Increased erectile dysfunction (with grafting procedures)
Sensory changes
Recurrence of curvature
Haematoma

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • When penile curvature or a hard plaque is noticed
  • When difficulty with intercourse develops
  • When pain on erection is present
  • For surgical evaluation once the disease has reached the stable phase

Treatment Methods

01
Plication procedure (curvature <60°, good erectile function)
02
Plaque incision and grafting (curvature >60° or hourglass deformity)
03
Penile prosthesis implantation (in associated severe ED)
04
Intralesional collagenase Clostridium histolyticum (CCH) injection
05
Traction therapy (preoperative or adjuvant)
