What is Penile Prosthesis Implantation?

Penile prosthesis implantation is the surgical placement of a prosthesis in the corpora cavernosa to produce an erection in severe organic erectile dysfunction that has not responded to oral therapy (PDE5 inhibitors), intracavernosal injection or vacuum devices.

Three-piece inflatable prostheses (AMS 700, Coloplast Titan) are the most widely used type and provide the most natural-looking erection. Malleable (semi-rigid) prostheses are a simpler but less natural alternative.

Patient and partner satisfaction rates exceed 90%. Infection is the most serious complication (1–3%); antibiotic-coated and hydrophilic-coated prostheses reduce the infection risk.

Symptoms

Severe organic erectile dysfunction unresponsive to oral therapy
Erectile dysfunction due to Peyronie's disease
Refractory erectile dysfunction after radical prostatectomy
Severe erectile dysfunction due to diabetes
Corporal fibrosis after priapism

Risk Factors

Prosthesis infection (1–3%)
Mechanical failure (5–15% at 10 years)
Erosion (through urethra or skin)
Perception of penile shortening
Haematoma

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • When erectile dysfunction is refractory to all therapies — surgical evaluation
  • If postoperative fever, skin redness or discharge develops (infection)
  • If difficulty inflating or deflating the prosthesis occurs (mechanical failure)
  • If penile pain, asymmetry or signs of erosion are noticed
