What is Intraprostatic Botulinum Toxin Injection?

Intraprostatic botulinum toxin injection is a treatment option for patients with lower urinary tract symptoms due to benign prostatic hyperplasia (BPH) who are unsuitable for surgery or unresponsive to medical therapy.

Botulinum toxin A (BTA) acts by relaxing prostatic smooth muscle and inducing apoptosis in prostatic gland tissue. It is injected into prostate lobes under transrectal ultrasonography or cystoscopy guidance. Generally 100-300 units are used.

The effect begins within 2-4 weeks and may last 6-12 months. Repeat injections may be needed. It is a good alternative for patients with high anesthesia risk, those on anticoagulants, and those who refuse surgery.

Symptoms

Difficulty in urination (BPH symptoms)
Frequent urination (pollakiuria)
Nighttime urination (nocturia)
Weak urinary stream
Urinary retention (chronic catheterized patients)

Risk Factors

Transient hematuria
Urinary infection (2-3%)
Inadequate response (in some patients)
Transient dysuria
Diminishing effect over time (need for repeat injection)
