What is Hypospadias Repair (Urethroplasty)?

Hypospadias is a congenital anomaly in male children in which the opening of the urethra (meatus) is located more proximally on the ventral surface of the penis than normal. Its incidence is 1/200-300 live male births. It is classified as distal (70%), midshaft (15%), and proximal (15%).

Surgical repair is usually performed as a single procedure between 6 and 18 months of age. TIP (Tubularized Incised Plate - Snodgrass), MAGPI, Mathieu, and onlay island flap are the most commonly used techniques. The aim is to bring the urethral meatus to the tip of the glans, correct chordee (ventral curvature), and obtain a cosmetically acceptable result.

In proximal hypospadias and severe chordee, staged repair (two-stage) may be required. In the first stage, the chordee is corrected and a graft is placed; in the second stage, the neourethra is created. Preoperative testosterone or DHT therapy may facilitate surgery by increasing penis size.

Symptoms

Opening of the urinary canal below the tip of the penis
Ventral curvature of the penis (chordee)
Dorsal hood prepuce (foreskin irregularity)
Downward direction of the urinary stream
Sexual dysfunction (in untreated adult cases)

Risk Factors

Urethrocutaneous fistula (5-15%)
Meatal stenosis (narrowing)
Urethral diverticulum
Residual chordee
Glans dehiscence
