What is Hydrocelectomy (Hydrocele Surgery)?

Hydrocele is an abnormal accumulation of fluid between the layers of the tunica vaginalis. Acquired (secondary) hydrocele is more common in adults and develops due to trauma, infection, tumor, or idiopathic causes. In children, the most common type is communicating hydrocele (a patent processus vaginalis).

On physical examination, the transillumination test is positive. Scrotal ultrasonography confirms the diagnosis and excludes underlying pathology such as a testicular tumor. In reactive hydrocele, treatment of the primary pathology is essential.

In surgery, the Jaboulay (eversion) and Lord (plication) techniques are most commonly used. In children, ligation of the processus vaginalis is performed via the inguinal approach. Aspiration and sclerotherapy are not preferred due to high recurrence rates.

Symptoms

Painless swelling in the scrotum
Sensation of scrotal heaviness
Diurnal variation in scrotal size
Difficulty walking or sitting (with large hydrocele)
Cosmetic discomfort

Risk Factors

Hematoma formation
Wound infection
Recurrent hydrocele (5-10%)
Testicular atrophy (rare, with vascular injury)
Chronic pain

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • When increasing scrotal swelling is noticed
  • Scrotal ultrasonography should rule out testicular tumor
  • If a painful or tense hydrocele develops
  • In children, when the hydrocele does not resolve spontaneously within 12-18 months
