What is Eustachian Tube Dysfunction?

The Eustachian tube connects the middle ear to the nasopharynx and equalises the pressure across the tympanic membrane during swallowing and yawning.

Obstructive dysfunction (most common) causes negative middle-ear pressure, effusion, and conductive hearing loss; patulous (abnormally open) dysfunction causes autophony.

Allergic rhinitis, viral upper respiratory infections, adenoid hypertrophy, smoking, and nasopharyngeal mass lesions are leading causes.

Diagnosis is clinical with otoscopy, tympanometry, audiometry, and nasopharyngeal examination to exclude obstructing pathology.

Symptoms

Ear fullness or pressure sensation
Decreased or fluctuating hearing
Ear pain (especially with altitude or pressure changes)
Clicking or popping sounds in the ear
Dizziness or imbalance

Risk Factors

Chronic allergic rhinitis
Frequent upper respiratory infections
Adenoid hypertrophy (in children)
Smoking
Barotrauma (air travel, scuba diving)

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • If ear fullness persists longer than two weeks
  • If hearing loss is noticed
  • If ear discharge develops

Treatment Methods

01
Topical nasal decongestants and intranasal corticosteroids
02
Antihistamines and treatment of underlying allergic rhinitis
03
Valsalva manoeuvre and auto-inflation exercises
04
Ventilation (tympanostomy) tube placement in chronic cases
05
Eustachian tube balloon dilation for refractory dysfunction
