What is Earwax Impaction (Cerumen Impaction)?

Cerumen is produced by ceruminous and sebaceous glands in the outer third of the external auditory canal and normally migrates outward.

Impaction occurs when wax fails to clear and hardens, frequently exacerbated by cotton-bud use, hearing aids, narrow canals, or advancing age.

Presentation is decreased hearing, ear fullness, pain, tinnitus, or occasionally dizziness.

Diagnosis is made by otoscopy; audiometry is reserved for persistent hearing loss after clearance.

Symptoms

Decreased or muffled hearing
Feeling of ear fullness or blockage
Ear pain
Tinnitus (ringing in the ear)
Dizziness (uncommon)

Risk Factors

Regular use of cotton buds or other objects in the ear
Narrow or tortuous external ear canal
Hearing-aid or in-ear earphone use
Advanced age (changes in cerumen consistency)
Use of swimming ear plugs

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • If hearing loss is noticeable
  • If home softening measures fail to clear symptoms
  • If ear pain or discharge develops

Treatment Methods

01
Cerumenolytic drops (hydrogen peroxide, sodium bicarbonate, olive oil)
02
Ear irrigation with warm water by a trained clinician
03
Manual removal with curette under direct vision
04
Microsuction under binocular microscopy (preferred for perforation risk)
05
Patient education on safe ear hygiene (avoid cotton buds)
