What is Cystocele Repair (Bladder Prolapse Surgery)?

Cystocele is the descent of the bladder into the anterior vaginal wall due to weakness of the pubovesical fascia and pelvic floor muscles. It is also known as anterior vaginal wall prolapse and is graded using the Baden-Walker or POP-Q classification.

Birth trauma, menopause, obesity, and chronic intra-abdominal pressure (chronic cough, constipation) are the main risk factors. In mild cases, pelvic floor exercises (Kegel) and pessary therapy may be applied.

In surgery, anterior colporrhaphy (anterior vaginal wall repair) is the most commonly used technique. In advanced prolapse, abdominal sacrocolpopexy (mesh suspension to the sacrum) provides more durable results. Robotic sacrocolpopexy is the current minimally invasive approach.

Symptoms

Sensation of bulging or mass from the vagina
Difficulty voiding (inability to empty the bladder)
Stress incontinence (urine leakage with cough or sneeze)
Pelvic pressure sensation
Recurrent urinary tract infections

Risk Factors

Recurrent prolapse (10-30%)
Mesh complications (erosion, pain - if mesh is used)
Stress incontinence (de novo or worsening)
Dyspareunia (painful intercourse)
Bleeding and infection
