What is Wide Resection with Tumor Megaprosthesis?

Wide resection with tumor megaprosthesis is a limb-salvage strategy used after oncologic wide excision of primary bone sarcomas (osteosarcoma, Ewing sarcoma, chondrosarcoma) or metastatic bone disease around the proximal femur, distal femur, proximal tibia, proximal humerus and pelvis.

Modular tumor endoprostheses allow tailored reconstruction after segmental resection with anatomic restoration of joint function, soft-tissue reattachment via Trevira tubes or biologic augmentation, and options for expandable implants in growing children.

Surgery is part of a multidisciplinary oncology pathway including neoadjuvant chemotherapy, advanced imaging, image-guided resection planning, perioperative infection prophylaxis and structured rehabilitation, with reported limb survival exceeding 80% at 5-10 years.

Symptoms

Persistent bone pain at rest
Pathologic fracture
Palpable mass in extremity
Functional limitation of adjacent joint
Progressive radiographic lytic or sclerotic lesion
Symptoms of metastatic bone disease
Constitutional symptoms in primary sarcoma

Risk Factors

Primary bone sarcoma diagnosis
Metastatic carcinoma to bone
Adjacent joint or neurovascular involvement
Pathologic fracture
Prior radiation therapy
Failed limb-salvage with conventional reconstruction
Recurrent disease after prior surgery

When to See a Doctor?

  • Symptomatic bone metastasis
