What is Vulvar Varicose Veins in Pregnancy?

Vulvar varicose veins are dilated, tortuous superficial vulvar venous plexuses caused by elevated pelvic venous pressure and progesterone-mediated venous wall relaxation during pregnancy.

Most appear in the second trimester and resolve spontaneously within six weeks postpartum.

Severe disease may be associated with deep pelvic vein incompetence (pelvic congestion syndrome) or rarely thrombophlebitis and bleeding.

Symptoms

Bluish, dilated and tender vulvar swelling
Heaviness and discomfort, worsening with prolonged standing
Pain during walking, sitting and intercourse
Itching, burning and skin irritation
Tense engorgement late in pregnancy
Rare bleeding after spontaneous rupture or trauma

Risk Factors

Multiparity and advancing maternal age
Family history of venous insufficiency
Lower limb varicose veins or pelvic congestion syndrome
Prolonged standing occupation
Obesity and excess weight gain in pregnancy

  • Active bleeding from vulvar varicosities
  • Persistent symptoms despite conservative management

Postpartum referral to vascular surgery for persistent symptoms beyond six months (sclerotherapy, venous ablation)
