Vulvar Lichen Sclerosus

What is Vulvar Lichen Sclerosus?

Lichen sclerosus is a chronic dermatosis with autoimmune mechanisms. It is most common in postmenopausal women but is also seen in prepubertal girls and young women. Characteristic 'figure-of-eight' whitening occurs in the vulvar, perineal and perianal area.

The disease typically presents with severe itching, burning, painful intercourse and, in advanced stages, distortion of the vulvar anatomy. Clitoral closure, labial fusion and narrowing of the introitus are important complications.

Although clinical appearance is the priority in diagnosis, biopsy is the gold standard. Highly potent topical corticosteroids (clobetasol propionate) are the first-line treatment. Long-term follow-up is important regarding vulvar intraepithelial neoplasia and squamous cell carcinoma.

Severe vulvar itching
Burning and pain
Porcelain-white skin plaques
Skin thinning and fragility
Pain during sexual intercourse
Loss of anatomical structures

Risk Factors

Female sex and postmenopausal period
Autoimmune diseases (thyroiditis, vitiligo)
Genetic predisposition
Traumatized areas (Koebner phenomenon)
Estrogen deficiency
Family history
