What is Indications for Vulvar Biopsy?

Vulvar biopsy is the removal of a small tissue sample from the vulva for histopathologic examination using punch, shave, or excisional techniques.

Indications include persistent pigmented lesions, ulcers not healing within four weeks, leukoplakic plaques, and any lesion suspicious for malignancy.

Common conditions diagnosed include vulvar squamous cell carcinoma, melanoma, vulvar intraepithelial neoplasia, lichen sclerosus, and chronic dermatitis.

The procedure is performed under local anesthesia in the office, with minimal complications and rapid healing.

Symptoms

Persistent vulvar itching or burning despite treatment
Pigmented or color-changed lesions raising malignancy concern
Ulcer or sore not healing within four weeks
White plaques (leukoplakia) on the vulva
Persistent rash unresponsive to topical therapy
Mass or thickened area on the vulvar skin

Risk Factors

Older age (postmenopausal women at higher risk for vulvar cancer)
HPV infection and history of cervical or vaginal dysplasia
Smoking, especially in HPV-positive patients
Lichen sclerosus (predisposes to differentiated VIN and cancer)
Immunosuppression (HIV, organ transplantation)
History of vulvar dysplasia or other anogenital cancer

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • New or changing pigmented vulvar lesion
  • Persistent vulvar pruritus despite topical treatment
  • Non-healing ulcer or wound on the vulva
  • Bleeding or discharge from a vulvar lesion
  • Lichen sclerosus with evolving thickening or ulceration
  • Postmenopausal vulvar lesion of any type warranting evaluation

Treatment Methods

Punch biopsy (3-4 mm) for most lesions in the office under local anesthesia

Shave biopsy for raised lesions when full thickness sample is unnecessary

Excisional biopsy for suspicious pigmented lesions or small discrete masses

Histopathologic examination by gynecologic or dermatopathologist

Specialized referral for confirmed malignancy or dysplasia

Follow-up management based on diagnosis (topical steroids, surgery, oncology referral)
