What is Vaginal Candidiasis (Thrush)?

Vulvovaginal candidiasis (VVC) affects about 75% of women at least once in their lives.

Candida albicans accounts for 90% of infections; non-albicans species (C. glabrata, C. tropicalis) may show azole resistance.

Antibiotic use, high oestrogen (pregnancy, COCPs), uncontrolled diabetes, and immunosuppression disrupt normal flora and promote overgrowth.

Recurrent VVC (RVVC) is defined as ≥4 symptomatic episodes per year and requires prolonged maintenance therapy after culture-guided species identification.

Symptoms

Intense vulvovaginal itching
Thick white curd-like discharge (no odour)
Burning and redness of the vulva
Burning during urination (external dysuria)
Dyspareunia (pain with intercourse)
Vulvar swelling and fissures

Risk Factors

Broad-spectrum antibiotic use
Pregnancy or high oestrogen states
Uncontrolled diabetes mellitus
Immunosuppression (HIV, corticosteroid therapy)
Tight synthetic underwear and prolonged wetness
Vaginal douching

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • With the first episode of candida
  • If over-the-counter treatment fails after one week
  • With 4 or more episodes per year
  • During pregnancy with any vaginal infection
  • If diabetes or an immune disorder is present

Treatment Methods

Topical azoles (clotrimazole, miconazole) for 1-7 days

Oral fluconazole 150 mg single dose (contraindicated in pregnancy)

Induction and maintenance for RVVC: weekly fluconazole for 6 months

Boric acid vaginal suppositories for non-albicans species

Correction of underlying factors (diabetes control, antibiotic stewardship)

Lactobacillus probiotic support
