Uterine Rupture

What is Uterine Rupture?

Uterine rupture is separation of all layers of the uterine wall (complete) or a partial form in which the serosa remains intact (dehiscence). The incidence is 1/20,000 in unscarred uteri and 0.5-1% in VBAC trials.

The clinical picture is characterized by sudden severe abdominal pain, abnormal fetal heart sounds (bradycardia), vaginal bleeding, and loss of the presenting part. The fetus may pass into the abdominal cavity.

Without emergency laparotomy, it leads to maternal and fetal death. Perinatal mortality is 50-75% and maternal mortality 1-13%.

Symptoms

Sudden severe abdominal pain (often at the cesarean scar)
Sudden fetal bradycardia or absent fetal heart
Vaginal bleeding
Hypotension and shock signs
Recession of the presenting part
Abdominal asymmetry and palpable fetal parts

Risk Factors

Previous cesarean (classical incision is more risky)
VBAC (trial of labor after cesarean)
History of uterine surgery (myomectomy)
High-dose oxytocin induction
Grand multiparity (>5 deliveries)
Trauma and placenta percreta
