What is Uterine Prolapse?

Uterine prolapse develops with insufficiency of the cardinal and uterosacral ligaments and pelvic floor muscles. In mild cases the uterus is partially descended from its normal position; in advanced cases the uterus may protrude outside the vagina (procidentia).

Birth trauma, advanced age, estrogen deficiency in menopause, chronic constipation, and cough are the main causes. Prolapse is often accompanied by other compartment defects such as cystocele and rectocele.

Treatment is determined by symptoms and impact on quality of life: pelvic floor exercises, pessary, local estrogen, and surgery (vaginal hysterectomy + colporrhaphy, sacrohysteropexy) are options.

Symptoms

Sense of swelling and fullness in the vagina
Sensation of pulling down in the pelvis
Lower back pain
Difficulty urinating or incontinence
Constipation and difficulty defecating
Discomfort during sexual intercourse

Risk Factors

Multiple vaginal births
Delivery of large babies and prolonged labor
Menopause and advanced age
Heavy work and lifting
Chronic cough and constipation
Obesity and connective tissue weakness

  • When swelling or palpable mass is felt in the vagina
  • When urinary incontinence and frequency are added
