What is Twin Pregnancy Complications?

Multiple pregnancies naturally occur at a rate of 1-2%, while this rate has increased with assisted reproductive techniques. Chorionicity (dichorionic-diamniotic, monochorionic-diamniotic, monochorionic-monoamniotic) determines the complication profile and must be detected in the first trimester.

Maternal complications include hyperemesis, preeclampsia, gestational diabetes, anemia, placenta previa, postpartum hemorrhage, and need for prolonged hospitalization. Thromboembolic risk is also increased.

Fetal and neonatal risks are preterm delivery, low birth weight, congenital anomalies, IUGR, TTTS, sIUGR, monoamniotic cord entanglement, and perinatal death. Frequent ultrasound, Doppler follow-up, and planned delivery reduce these risks.

Symptoms

Uterine size larger than expected for gestational age
Severe nausea and vomiting (hyperemesis)
Early and significant maternal weight gain
Fetal movements felt in different regions
Increased shortness of breath
Edema in the legs

Risk Factors

IVF and ovulation induction
Advanced maternal age
Family history of multiple pregnancy
Previous history of multiple pregnancy
Monochorionic placentation
Concurrent medical illnesses in multiple pregnancies

  • Perinatology evaluation when multiple pregnancy is detected
  • When there are regular contractions or watery discharge
  • When severe headache and visual blurring develop
  • When decrease in fetal movements is noticed
  • Sudden rises in blood pressure
