What is Tube Thoracostomy (Chest Tube)?

Tube thoracostomy is the placement of a catheter into the pleural cavity for pleural pathologies such as pneumothorax, hemothorax, empyema, and chylothorax.

It is usually performed in the midaxillary line at the 4th or 5th intercostal space. The tip of the tube is placed in the pleural space and connected to a water-seal system.

It is frequently applied in trauma, spontaneous pneumothorax, pleural effusions, and after thoracic surgery. Performing it at the correct anatomical site under sterile conditions is essential.

Symptoms

Pneumothorax symptoms (shortness of breath, pleuritic pain)
Hemothorax symptoms (hypotension, hypoxia)
Pleural effusion (dyspnea, cough)
Empyema (fever, cough, chest pain)
Trauma-related lung injury
Barotrauma during mechanical ventilation

Risk Factors

Chest trauma
Chronic obstructive pulmonary disease
Pneumonia and parapneumonic effusion
History of thoracic surgery
Long-term mechanical ventilation
Malignancy (pleural involvement)

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • The procedure is performed only by emergency medicine, thoracic surgery, or trauma physicians
  • Family members should be informed about possible complications
  • A chest X-ray should be taken after the procedure to confirm tube position
  • Timely removal of the tube reduces the risk of infection

Treatment Methods

01
Patient positioning and sterile field preparation
02
Local anesthetic application
03
Blunt dissection through the intercostal space
04
Placement of the tube into the pleural cavity
05
Connection to a water-seal system and securing
06
Daily drainage monitoring and follow-up X-rays
