Trigger Finger (Stenosing Tenosynovitis)

What is Trigger Finger (Stenosing Tenosynovitis)?

Trigger finger (stenosing tenosynovitis) is the narrowing of the sheath surrounding the flexor tendon of the finger (at the A1 pulley ligament region) causing the tendon to be unable to pass through this narrow area. A 'click' sound is heard as the finger is flexed, or the finger locks.

It is more common in middle-aged and older women. Trigger finger affecting multiple fingers is a common complication in diabetic patients. The thumb, middle finger, and ring finger are more commonly affected.

The finger may be in a locked position when waking up in the morning; it can be opened by hand but is painful. In advanced stages, permanent flexion contracture can develop.

Symptoms

'Click' sound or locking when bending the finger
Morning stiffness and rigidity
Pain and tenderness at the finger base
Difficulty straightening the finger (sometimes requiring manual opening)
Finger remaining fixed in flexion in advanced stages
Sensation of swelling at the finger base

Risk Factors

Diabetes (risk of multiple trigger fingers)
Rheumatoid arthritis
Work or hobbies requiring repetitive gripping
Hypothyroidism
Female sex and middle age
Coexistence with carpal tunnel syndrome

  • When the finger locks and you cannot open it yourself
  • If the finger can no longer straighten
  • If morning stiffness affects daily life
  • If a child's thumb is locking
  • If you have diabetes and multiple fingers are affected
