What is Triangular Fibrocartilage Complex (TFCC) Tear?

The triangular fibrocartilage complex (TFCC) is a complex structure on the ulnar side of the wrist between the distal radius and ulna, consisting of the central disc, ulnar collateral ligament, palmar and dorsal radioulnar ligaments, and the ECU tendon sheath.

It is divided into type 1 (traumatic) and type 2 (degenerative) according to Palmer classification. The tear leads to ulnar-sided wrist pain, clicking, and loss of grip strength.

Ulnar fovea sign, press test, and TFCC stress test are used in clinical diagnosis. MR arthrography is the gold standard. Arthroscopy can be used for diagnosis and treatment.

Symptoms

Ulnar-sided wrist pain
Pain in gripping and twisting movements
Clicking in the wrist
Decreased grip strength
Restriction in forearm rotation
Ulnar fovea tenderness
Distal radioulnar joint instability

Risk Factors

Falling on outstretched hand
Ulnar positive variance
Distal radius fracture
Repetitive grasping activities
Racket sports
Advanced age degeneration
Golf and tennis
