What is Tarsal Tunnel Syndrome?

The tarsal tunnel is a canal on the medial side of the ankle, formed between the fibrous retinaculum behind the medial malleolus and the underlying bone. The posterior tibial nerve, flexor tendons, and vessels pass through this canal. Compression of the nerve here results in tarsal tunnel syndrome.

Scar tissue after ankle fracture or sprain, ganglion cysts, varicose veins, flatfoot, and systemic diseases (rheumatoid arthritis, hypothyroidism, diabetes) can cause nerve compression.

It can be regarded as the foot equivalent of carpal tunnel syndrome, although it is not as common.

Symptoms

Burning and tingling in the sole and toes
Pain that increases with prolonged standing
Night pain
Positive Tinel's sign (numbness on tapping behind the medial malleolus)
Intrinsic muscle weakness (in advanced cases)
Decreased sensation in the sole

Risk Factors

Flat foot (pes planus)
History of ankle fracture or sprain
Ganglion cyst or mass
Diabetes and hypothyroidism
Rheumatoid arthritis
Varicose veins

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • For chronic burning or numbness in the sole
  • For pain limiting prolonged standing
  • For evaluation with nerve conduction studies
  • For lack of response to conservative treatment

Treatment Methods

01
Conservative: orthotic insoles, shoe modification
02
NSAIDs and neuropathic pain medications
03
Corticosteroid injection
04
Physiotherapy and nerve mobilization exercises
05
Surgical tarsal tunnel decompression
06
Treatment of the underlying cause
