Talar Neck Fracture

What is Talar Neck Fracture?

The talus is a key bone of the ankle and hindfoot skeleton. The neck is the most frequently fractured site of the talus and the injury usually results from forced ankle dorsiflexion.

More than 60% of the talar surface is cartilage and the bone is fed by external vessels. Disrupted blood supply after fracture leads to a 20-50% risk of avascular necrosis (bone death).

The Hawkins classification divides the injury into four types and the risk of avascular necrosis rises with each type. Early anatomic reduction and stable fixation reduce the avascular necrosis risk.

Symptoms

Severe ankle pain and swelling
Inability to bear weight on the ankle
Bruising over the ankle and hindfoot
Marked limitation of ankle motion
Ankle deformity (in severe fractures)

Risk Factors

Fall from height
Motor vehicle accidents
Sports injuries (especially parachuting and snowboarding)
Forced dorsiflexion of the ankle

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • When weight bearing is impossible after ankle trauma
  • When marked swelling and deformity of the ankle occur
  • When signs of impaired foot circulation appear

Conservative short-leg cast in non-displaced fractures

Urgent open reduction and screw fixation in displaced fractures

Protective treatment and weight restriction if avascular necrosis develops

Talonavicular or tibiotalar arthrodesis in advanced necrosis

Long-term rehabilitation and graded weight-bearing program
