What is Severe Burn Emergency Management?

A severe burn is a second-degree or higher burn affecting a significant portion of the body surface area (>20% in adults, >10% in children). It carries serious risks of fluid loss, hypovolemia, infection, and respiratory damage.

It can be caused by heat, chemicals, electricity, or radiation. Airway burns and inhalation injuries increase mortality.

Fluid resuscitation using the Parkland formula, an early decision on intubation, and referral to a burn center are core principles.

Symptoms

Extensive skin damage and blistering
Painless dark-colored tissue in deep burns
Soot and singed hair on the face and neck
Stridor and hoarseness (inhalation)
Tachycardia and hypotension
Confusion and weakness

Risk Factors

House fires and gas cylinder explosions
Industrial accidents and welding burns
Electric shock
Exposure to chemicals
Burns from falls in patients with epilepsy
Hot liquid scalds in children

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Call 112 for burns affecting the face, hands, feet, genitals, or joints
  • Electrical burns must always be evaluated in hospital
  • Make an emergency call for any breathing difficulty or voice change
  • Apply without delay for extensive second-degree or higher burns

Treatment Methods

Removal from the burn source and elimination of the cause

Fluid therapy with lactated Ringer's per the Parkland formula

Cover wounds with clean dressings and sterile bandages

Pain control and tetanus prophylaxis
