What is Serotonin Syndrome — Emergency?

Serotonin syndrome (also called serotonin toxicity) is a potentially life-threatening adverse drug reaction resulting from excessive activity of serotonin in the central and peripheral nervous systems, most often when two or more serotonergic agents are combined or doses are escalated.

Implicated drugs include selective serotonin reuptake inhibitors, serotonin-norepinephrine reuptake inhibitors, monoamine oxidase inhibitors, tricyclic antidepressants, tramadol, linezolid, methylene blue, dextromethorphan, ondansetron, MDMA and herbal supplements such as St John wort.

The clinical triad encompasses mental status changes (anxiety, agitation, delirium), autonomic hyperactivity (tachycardia, hypertension, diaphoresis, hyperthermia, mydriasis) and neuromuscular abnormalities (clonus particularly inducible at the ankles, tremor, hyperreflexia and rigidity).

Diagnosis is clinical (Hunter or Sternbach criteria); exclusion of neuroleptic malignant syndrome, anticholinergic toxicity and malignant hyperthermia is essential. Hyperthermia greater than 41 degrees, severe muscle rigidity and rhabdomyolysis define life-threatening disease.

Treatment requires immediate cessation of all serotonergic agents, aggressive cooling, benzodiazepines for agitation and tremor, intravenous fluids, and cyproheptadine (a serotonin antagonist) in moderate to severe cases; intensive care monitoring and intubation with paralytics may be needed.

Symptoms

Agitation, anxiety and confusion
Tachycardia and hypertension
Diaphoresis and shivering
Hyperthermia (often greater than 38 degrees)
Inducible or spontaneous lower limb clonus
Hyperreflexia greater in lower than upper limbs
Tremor and muscle rigidity
Mydriasis and hyperactive bowel sounds
Seizures and rhabdomyolysis in severe cases

Risk Factors

Combination of two or more serotonergic agents
Dose escalation or recent initiation of serotonergic drug
Use of monoamine oxidase inhibitors with other serotonergics
Linezolid or methylene blue with serotonergic antidepressants
Recreational use of MDMA or LSD in serotonergic context
Tramadol or fentanyl in patient on antidepressant
Herbal supplements such as St John wort or ginseng

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Agitation with tremor after starting or increasing antidepressant dose
  • Fever with sweating and rapid heart rate on serotonergic drugs
  • Muscle stiffness or jerking after combination drug use
  • Confusion or seizure on multiple serotonergic agents
  • Sudden behavioral change with high temperature on antidepressants

Treatment Methods

Immediate discontinuation of all serotonergic agents

Aggressive external cooling and intravenous fluids

Benzodiazepines (diazepam, lorazepam) for agitation and tremor

Cyproheptadine in moderate to severe cases (loading 12 mg then 2 mg every 2 hours)

Endotracheal intubation and non-depolarizing paralysis (avoid succinylcholine) in severe rigidity

Avoidance of bromocriptine, dantrolene and antipyretics

Treatment of rhabdomyolysis and acute kidney injury

Monitoring in intensive care unit until autonomic stability
