What is Septic Shock?

Septic shock is a clinical syndrome of circulatory failure and tissue hypoperfusion resulting from a widespread inflammatory response to infection.

It is defined by hypotension that is unresponsive to fluid therapy and elevated lactate levels. Mortality ranges between 30% and 50%.

Early antibiotics, fluid resuscitation and source control are life-saving. The bundle delivered in the first hour determines prognosis.

Symptoms

High fever or hypothermia
Tachycardia and hypotension
Tachypnoea and altered consciousness
Cold sweating and mottled skin
Oliguria and weakness
Findings related to the source of infection (dysuria, cough, wound discharge)

Risk Factors

Advanced age, immunosuppression
Chronic illness (diabetes, chronic kidney disease, cirrhosis)
Invasive procedures and catheters
Recent surgery or hospitalisation
Chemotherapy and immunosuppressive treatment
Malnutrition

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Call emergency services if high fever is accompanied by hypotension
  • A febrile patient with confusion and weakness must be brought to the emergency department
  • Mandatory presentation when a patient with a known infection deteriorates
  • Even mild symptoms must be taken seriously in immunocompromised patients

Treatment Methods

01
Blood and urine cultures
02
Empirical broad-spectrum antibiotics in the first hour
03
Crystalloid resuscitation at 30 mL/kg
04
Vasopressor support (noradrenaline first choice)
05
Lactate monitoring and goal-directed therapy
06
Source control of the infection (drainage, debridement)
