What is Sentinel Lymph Node Biopsy in Gynecologic Oncology?

Sentinel lymph node (SLN) mapping identifies the first lymph node receiving lymphatic drainage from the tumor. Indocyanine green (ICG), methylene blue, or technetium-99m radiocolloid can be used.

In endometrial cancer, near-infrared camera-guided SLN detection with cervical injection has become standard. The FIRES and SENTI-ENDO trials demonstrate the reliability of the technique.

In vulvar cancer, SLN can be used in early-stage (T1b <4 cm) patients as an alternative to full inguinofemoral lymphadenectomy. Research is also ongoing in cervical cancer.

High-grade histology
Mid/deep cytological localization
Tumor size
Long-term chronic lymphedema risk
