Scaphoid Fracture

Fracture of the most commonly broken carpal bone, prone to healing problems due to limited blood supply.

What is Scaphoid Fracture?

The scaphoid (navicular) is a carpal bone on the thumb side of the wrist that bears the greatest load during wrist motion. Its proximal pole has poor blood supply and is particularly prone to healing problems.

It accounts for 70-80% of carpal fractures from a fall onto an outstretched hand. Tenderness in the 'anatomical snuffbox' is the typical finding. It may not be visible on initial radiographs; MRI or CT is used for early diagnosis.

Delayed or untreated scaphoid fractures can lead to nonunion, avascular necrosis from vasospasm, and long-term wrist arthritis known as SNAC (Scaphoid Nonunion Advanced Collapse).

Symptoms

Wrist pain on the thumb side
Tenderness in the anatomical snuffbox (diagnostic finding)
Pain when making a fist or gripping
Restricted wrist motion and swelling
May be invisible on radiograph (in the first 2 weeks)

Risk Factors

Fall on an outstretched arm
Sports injuries (gymnastics, skiing)
More common in young male athletes
Loading with wrist dorsiflexion

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Pain on the thumb side after a wrist fall
  • MRI evaluation when there is clinical suspicion despite normal radiographs
  • Suspected nonunion after treatment
  • Early treatment: delay increases avascular necrosis risk
