What is Robotic Radical Hysterectomy?

Robotic radical hysterectomy is a minimally invasive surgical method in gynecologic oncology that allows removal of the uterus and adjacent supporting tissues. The da Vinci system offers high-resolution 3D imaging, instrument degrees of freedom, and tremor filtration.

The LACC trial reported higher recurrence and worse survival outcomes with minimally invasive radical hysterectomy, making careful patient selection mandatory. Techniques that minimize tumor manipulation and recommendations for intracorporeal colpotomy are included in current protocols.

Preoperative evaluation includes MRI for local extent, PET-CT for distant metastasis screening, and gynecologic oncology and radiation oncology consultations. Pelvic lymph node dissection or sentinel lymph node biopsy is part of the standard approach.

Symptoms

Diagnosis of early-stage cervical cancer
Suspicion of microscopic parametrial invasion
High-risk subtypes of endometrial cancer
Localized gynecologic malignancy
Fertility-sparing candidates for radical trachelectomy
Recurrent cervical lesion (selected)

Risk Factors

Tumor diameter >2 cm
Deep stromal invasion
Lymphovascular invasion
High-grade endometrial cancer
Pelvic obesity
Previous pelvic surgery

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • When colposcopy and biopsy are needed after an abnormal smear
  • When parametrial extension is seen on MRI
  • For a gynecologic oncology tumor board decision
  • When you want to discuss fertility-sparing options

Treatment Methods

01
Preoperative staging (MRI, PET-CT)
02
Robotic radical hysterectomy (Querleu-Morrow Type C)
03
Sentinel lymph node biopsy or pelvic dissection
04
Parametrial resection and upper vaginectomy
05
Postoperative pathologic risk classification
