What is Revision THA with Acetabular Bone Defect?

Acetabular bone defects in revision total hip arthroplasty (THA) result from osteolysis, infection, periprosthetic fracture and loosening; the Paprosky classification (I, IIA-C, IIIA-B) guides reconstructive strategy.

Reconstruction options include hemispherical jumbo cups with screws for contained defects, modular trabecular metal (tantalum) augments, structural allograft, antiprotrusio cages with cemented liners and patient-specific custom triflange components for severe pelvic discontinuity.

Surgical success depends on accurate preoperative templating, intraoperative defect assessment, restoration of hip center, secure ingrowth fixation, dual-mobility or constrained liner choice when stability is at risk and structured rehabilitation in high-volume centers.
