What is Rapid Sequence Intubation (RSI)?

Rapid Sequence Intubation (RSI) is the standard airway management technique used in the emergency department in patients assumed to have a full stomach in order to prevent aspiration.

Preoxygenation is performed first; a sedative (etomidate, ketamine, propofol) and a paralytic (succinylcholine, rocuronium) are then administered simultaneously to shorten the apnea period.

RSI is essential in patients with altered consciousness, respiratory failure or high aspiration risk. It requires an experienced team and appropriate equipment.

Inability to protect the airway
Oxygenation failure

Risk Factors

Full stomach (patient requiring emergency surgery)
Trauma and possible cervical injury
Obesity and predicted difficult intubation
Hypotension and shock
Hypoxemia
Pediatric patients

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • RSI decision is made only by emergency, anesthesia or intensive care physicians
  • Informing relatives for consent is important
  • Transfer to intensive care should be planned after the procedure
  • An alternative airway plan (LMA, surgical airway) should be ready
