What is Puerperal Sepsis?

Puerperal sepsis is the progression of genital tract infection (endometritis, parametritis, pelvic abscess) in the postpartum period to systemic inflammatory response syndrome (SIRS) or sepsis. It accounts for 10-15% of maternal deaths worldwide.

The most common etiologies include group A streptococcus, E. coli, Klebsiella, and anaerobes. Risk factors include prolonged membrane rupture, multiple vaginal examinations, cesarean section, and low socioeconomic status.

Rapid diagnosis, fluid resuscitation, and initiation of broad-spectrum IV antibiotics are critical. Development of septic shock raises mortality to 30%.

Symptoms

Fever ≥38°C (after 24 hours postpartum)
Lower abdominal pain and uterine tenderness
Foul-smelling, purulent lochia
Tachycardia, tachypnea
Hypotension, oliguria (sepsis)
Confusion, malaise

Risk Factors

Cesarean delivery
Prolonged membrane rupture (>18 hours)
Multiple vaginal examinations
Manual removal of placenta
Obesity and diabetes
Low socioeconomic status and anemia

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • When postpartum high fever occurs
  • With severe abdominal pain or uterine tenderness
  • With foul-smelling vaginal discharge
  • With tachycardia, hypotension, and weakness
  • With change in consciousness (urgent)

Broad-spectrum IV antibiotics (clindamycin + gentamicin + ampicillin)

Curettage if retained placenta
