What is POTS Postural Orthostatic Tachycardia Syndrome?

Postural Orthostatic Tachycardia Syndrome (POTS) is a chronic autonomic disorder defined as an excessive increase in heart rate (>30 bpm in adults, >40 bpm in adolescents) within 10 minutes of standing without orthostatic hypotension. Symptoms must persist for more than 6 months.

It is most often seen in young women aged 15-50 and may follow viral infections, trauma, surgery or pregnancy. Recently many post-COVID-19 cases of POTS have been reported.

Subtypes include neuropathic, hyperadrenergic and hypovolaemic forms, and treatment is tailored to the subtype.

Symptoms

Palpitations
Dizziness on standing
Pre-syncope or syncope
Severe fatigue and exercise intolerance
Brain fog and difficulty concentrating
Headache
Nausea and gastrointestinal symptoms

Risk Factors

Female sex (4-5x risk)
Age 15-50 years
Recent viral infection (especially COVID-19)
Joint hypermobility (Ehlers-Danlos)
Mast cell activation syndrome
Family history of autoimmune disease
Recent trauma, surgery or pregnancy

  • Recurrent syncope or pre-syncope
  • Symptoms severe enough to limit daily living
  • Persistent palpitations
  • Severe fatigue and brain fog
