Postpartum Venous Thromboembolism

What is Postpartum Venous Thromboembolism?

Postpartum venous thromboembolism includes deep vein thrombosis and pulmonary embolism seen within the first 6 weeks after delivery. Hypercoagulability in pregnancy, venous stasis from uterine pressure, and endothelial damage form Virchow's triad.

It is most commonly seen as left leg deep vein thrombosis, pelvic vein thrombosis, and pulmonary embolism. Cesarean delivery, prolonged immobilization, obesity, and thrombophilias increase the risk.

Symptomatic patients are started on full-dose low molecular weight heparin, continued for 3-6 months. Antenatal and postpartum anticoagulation at prophylactic doses is indicated in high-risk pregnant women.

Symptoms

Unilateral swelling, pain, redness in the calf
Pain in the leg when walking
Sudden shortness of breath and chest pain
Tachycardia and hypotension
Hemoptysis
Unexplained syncope

Risk Factors

Cesarean and emergency delivery
Preeclampsia and postpartum hemorrhage
Obesity (BMI >30)
Thrombophilia (Factor V Leiden, prothrombin mutation)
Advanced maternal age and multiple pregnancy
Prolonged bed rest

When to See a Doctor?

  • When unilateral leg swelling and pain occur after delivery
  • When sudden chest pain and shortness of breath begin
  • When dizziness or tachycardia develops
  • Emergency presentation in case of coughing up blood
  • For prophylaxis planning in high-risk groups
