What is Posterior Shoulder Dislocation?

Posterior shoulder dislocation accounts for only 2-4% of all shoulder dislocations but can be missed at initial evaluation in up to 50% of cases. On AP radiograph, it appears with the arm in adduction and internal rotation position.

Classic mechanisms are generalized seizures (epileptic seizure), electrical shock, electroshock therapy, and posterior-directed high-energy trauma. It can be bilateral.

A reverse Hill-Sachs lesion in the humeral head and a reverse Bankart lesion may accompany. If early diagnosis and reduction are unsuccessful, open reduction, subscapularis transfer, or rotational osteotomy may be required.

Symptoms

Inability to externally rotate the arm
Shoulder deformity (anterior beak, posterior fullness)
Severe shoulder pain
Range of motion limitation
Holding the arm in adduction and internal rotation
Coracoid process prominence
Palpable mass in the shoulder (posterior)

Risk Factors

Epileptic seizures
Electric shock
Electroconvulsive therapy
High-energy trauma
Alcoholism and hypoglycemia
Being a young man
Previous posterior instability

When to See a Doctor?

  • Development of shoulder pain after a seizure
  • Noticing deformity in the shoulder
  • Limitation in arm external rotation
  • Pain in shoulder movements after trauma
  • Emergency department admission is required
