Post-Cesarean Care

What is Post-Cesarean Care?

Since cesarean involves incisions in the abdominal wall and uterus, complete recovery usually takes 6-8 weeks. On average, a 3-5 day hospital stay is required; during this time, pain control, mobilization, wound care, and breastfeeding support are provided.

Weakness in the abdominal wall muscles may develop after a cesarean incision; in the first weeks, heavy lifting, climbing stairs, and driving should be restricted. Early ambulation and adequate fluid intake are recommended for the resumption of bowel movements.

Factors affecting wound healing include diabetes, obesity, anemia, and steroid use. Long-term considerations include cesarean scar depression (niche), bladder and bowel adhesions, and placenta accreta risk in the next pregnancy.

Symptoms

Pain, tenderness, and numbness at the incision site
Gas and decreased bowel movements in the first days
Lochia (post-delivery vaginal discharge): red in the first weeks, turning pink and decreasing over time
Fatigue and signs of anemia
Mild shoulder pain (gas pain referred to the diaphragm)

Risk Factors

Obesity and diabetes (slow wound healing)
Anemia (due to blood loss)
Immune system suppression
Emergency cesarean (higher complication risk than planned cesarean)
Previous abdominal surgery and adhesions

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • When redness, swelling, discharge, or fever at the incision site occurs
  • When severe abdominal pain or excessive vaginal bleeding is experienced
  • When leg swelling or pain (sign of deep vein thrombosis) develops
  • When breastfeeding difficulty is experienced
  • At the 6-week postpartum check-up appointment

Treatment Methods

Pain management: combination of paracetamol, NSAIDs, and opioid when needed

Incision care: keeping it dry, appropriate dressing, and early suture removal

Deep vein thrombosis prophylaxis: early mobilization, compression stockings, LMWH

Breastfeeding position counseling: football hold to reduce incision pressure

Physiotherapy to strengthen abdominal muscles (can be started after 6 weeks)

Recommendation of at least 18 months interval for the next pregnancy
