What is Point-of-Care Ultrasound in the Emergency Department?

POCUS (Point-of-Care Ultrasound) is bedside ultrasound performed by the clinician directly in the emergency department. It enables immediate clinical decision-making by addressing focused diagnostic questions in critical patients.

Common applications: FAST exam in trauma (free fluid evaluation), RUSH protocol in shock (rapid ultrasound for shock and hypotension), cardiac function evaluation, lung ultrasound (pneumothorax, pleural effusion), abdominal aorta evaluation, deep vein thrombosis screening.

It is also used for procedural guidance: central venous catheterization, paracentesis, thoracentesis, lumbar puncture, peripheral nerve blocks. Reduces complication rates, increases first-attempt success and improves patient safety.

Symptoms

Need for trauma evaluation (FAST)
Shock and hypotension etiology
Acute dyspnea diagnostic differentiation
Acute abdominal pain etiology
Suspicion of cardiac tamponade
Procedural guidance need (central catheter)

Risk Factors

Operator-dependent variability
Image quality limitations
Difficulty in obese patients
False-negative results
Need for adequate training
Inadequate quality assurance
